Memorable Heroic Endings Part 3.   

Its always sad when your psychic powers fail you. This happened in season 2 episode 6 .2 of Cthulhu DMD. My favorite game of 2021.

It was the last episode fatefully entitled endgame. Hailia failed to predict the ferocity of a newly summoned Yogsothoth. The experienced octogenarians last words are thought to be, "Something wicked this way comes". In actuality it arrived from that way and not this way as she had predicted. Fellow occult hunters  present at the time have spoken how possibly painless her demise may have been. Below is a reconstruction of the event.
